Residents, healthcare providers and community partners across Columbia and Hamilton Counties are being called on to share their perspectives on local health needs through a Community Health Assessment survey now open through Tuesday, March 31.
The survey, conducted by the WellFlorida Council, will help guide the development of a Community Health Needs Assessment. According to the website, the report will identify “the most pressing health issues in the region and serve as a tool for community leaders to take action.”
Two versions of the survey are available — one for residents and another for healthcare providers and community partners. Both surveys are offered in English and Spanish and take approximately 10 minutes to complete.
Residents of Columbia and Hamilton Counties who are at least 18 years old can participate in the community survey, which includes 10 core questions and additional items based on responses. Surveyors remain anonymous, and participants are asked to complete it only once.
Healthcare providers, social service professionals and community partners who serve the region are invited to complete a separate survey tailored to their experience. That version includes eight core questions, along with demographic items, and is also anonymous.
Community members can access the survey at wellfl.info/ResidentCH, while providers and partners can participate at wellfl.info/ProviderCH.
